To better serve the South Charlotte region with comprehensive, convenient care, effective August 31, 2025, we will be combining the teams at Avance Care Waxhaw and Avance Care Rea Farms, just 5 miles up Providence Road.

Avance Care Rea Farms will offer care for the whole family, and providers will be welcoming new patients of all ages! With extended hours, from 7:30 AM – 6:00 PM, you’ll be able to get care for the whole family, in one place, at a time that works for you.

Dr. Jeffrey Hutchings and his team of primary care providers, Mackenzie Coleman, PA-C and Derek Whalen, PA-C will be relocating to Rea Farms and will begin seeing patients on September 2, 2025. If you have appointments scheduled with them at Waxhaw after August 31st, a member of our team will contact you to reschedule your appointment at Avance Care Rea Farms.

Kara C. Mild, DSW, MSSA, LCSW will be relocating to Rea Farms.

Jovanna Orozco, MS, RDN, LDN will be leaving Avance Care on August 5, 2025. Rose Ashline, RDN, LDN at Rea Farms has availability to work with any of Jovanna’s patients not already scheduled with other Dietitians.

If you transfer your care to the Avance Care Rea Farms practice your medical records will automatically be available at the new location. By staying within the Avance Care network, your patient portal access will remain the same and your medical records will follow you automatically – no new forms to complete.

If you choose to transfer your care to a provider outside of Avance Care, you may request your medical records be sent to your new provider or directly to you.
