Jean Hammer, DNP, FNP-BC (she/her), is a board-certified Family Nurse Practitioner providing care at Avance Care North Wake Forest in Wake Forest, North Carolina.
Jean earned both her Master of Science in Nursing Education and her Doctor of Nursing Practice from the University of North Carolina at Chapel Hill. During her doctoral studies, she was selected as a Behavioral Health Integration Scholar, focusing on the integration of mental health care into primary care settings for common conditions such as ADHD, anxiety, and depression.
In addition to her clinical practice, Jean serves as a Clinical Assistant Professor at the UNC-Chapel Hill School of Nursing. Originally from San Diego, California, Jean began her nursing career on the West Coast before relocating to North Carolina, where she provided care for pediatric and adult oncology patients at Duke University Health System.
Jean’s clinical interests include cancer prevention, heart health, behavioral health, sexual and reproductive health—including family planning, menopause, perimenopause, PrEP, and gender-affirming care—and chronic condition management. She is especially passionate about supporting young adults and LGBTQ+ individuals, and she is committed to making high-quality, affordable healthcare accessible for all.
Outside of work, Jean enjoys learning Spanish, listening to podcasts, and relaxing with an iced coffee or hot tea. She also loves spending time outdoors with her two dogs, Moon and Theo, and sharing quality moments with friends and family. Her professional affiliations include the North Carolina Nurses Association, American Association of Nurse Practitioners, American Public Health Association, and the Oncology Nursing Society.
