MyChart Transition
During the transition to MyChart you will continue to have access to some functions of Avance Care’s legacy patient portal. These features include online bill paying (for statements prior to 9/20/25) and medical records requests. These functions will be live for 6 months after the transition to MyChart.
Effective 9/20/25 the legacy portal will have limited functionality and will no longer be used for messaging providers, requesting prescription refills, or viewing upcoming appointments.
