With this consolidation, effective 8/31/2025

Ike Nwanonyiri, MD and Destiny Walters, PA-C will no longer be seeing patients at Avance Care. If you are scheduled for an appointment with either provider after 8/31/2025, we will be reaching out to reschedule your appointment with another provider at the Rea Farms location. You can also call us directly at 980.432.1027 to reschedule your appointment.

Ann Mathew, MSN, FNP-C and Rose Ashline, RDN, LDN will be relocating to Avance Care Rea Farms. If you have appointments scheduled with them at Matthews after 8/31/2025, those appointments will automatically be transferred to the Rea Farms location.

Sausha Lotharp, LMHCA, NCC will continue seeing Behavioral Health patients from Avance Psychiatry in Charlotte. There will be no change for this provider.

If you transfer your care to the Avance Care Rea Farms practice your medical records will automatically be available at the new location. If you choose to transfer your care to a provider outside of Avance Care, you may request your medical records be sent to your new provider or directly to you.
