Dear Patients of Avance Care West Cary and Morrisville,

We have important news to share with you. Effective August 1, 2025, the primary care providers at our West Cary and Morrisville locations will no longer be part of the Avance Care family. As a result of this change, you have an important decision to make about your future care.

This change only affects patients who are currently seeing the following providers for primary care. These providers will be part of McCrimmon Primary Care Plus, which is completely independent of Avance Care. McCrimmon Primary Care Plus will continue at the same West Cary and Morrisville locations.

What This Means for Your Primary Care
Your choice of healthcare provider rests solely with you. If you are currently seeing one of the providers listed above for primary care, you will need to decide how you’d like to continue your care.

Stay with your current provider at the new practice, McCrimmon Primary Care Plus. No action is needed. They will have your primary care records on a go forward basis.

Choose a new Avance Care primary care provider at a nearby location and call or schedule online as usual.

Transfer to a provider outside of Avance Care or McCrimmon Primary Care Plus. Simply request your Avance Care records be transferred using the form found on the Medical Records section of our website.

Other Avance Care Services
If you receive other services at Avance Care (such as behavioral health, psychiatry, nutrition, or neurology), you can continue to receive those services either at another Avance Care location or virtually, regardless of where you receive your primary care in the future. The one exception is chronic care management (CCM). If you currently receive CCM services through Avance, you will receive a separate letter with additional information.
