HOW TO OPT OUT OF AVANCE CARE SUBMITTING YOUR INFORMATION TO HEALTH INFORMATION EXCHANGES

Option 1:
Call 980-375-0053 to leave a secure voicemail with your full name, phone number, and date of birth. Please be aware this phone number is an unattended phone line, but voicemails will be received and addressed. Upon receipt of your voicemail, we will contact you to confirm that you have been opted out. We will send you the Avance Care Health Information Exchange Opt Out Form in a self-addressed, stamped envelope for you to endorse your Opt Out in writing for our records.

Option 2:
Email [email protected] with your full name, phone number on file with Avance Care, and email address on file with Avance Care with subject line “Opt Out”. Upon receipt of your email, we will contact you to confirm that you have been opted out. We will send you the Avance Care Health Information Exchange Opt Out Form in a self-addressed, stamped envelope for you to endorse your Opt Out in writing for your medical records.
